Clinical guidelines recommend the use of suicide risk assessment tools in the emergency department (ED) setting. Several tools have been studied and validated for screening and evaluating patients at risk for self-harm or suicide. These tools include the ASQ, MSHR, RSQ, PSS, BHS-ED, C-SSRS, ReACT Self-Harm Rule, SAD PERSONS Scale, and SIS. The ASQ has shown high sensitivity and specificity for identifying at-risk pediatric and young adult patients. The MSHR, RSQ, PSS, and C-SSRS have also shown promise. However, these tools should not replace clinical judgment and factors like patient population and available resources should be considered when choosing a tool.
