This document is a clinical practice guideline on fall risk assessment in emergency department (ED) patients across the lifespan. Falls in hospital settings are common and can lead to significant morbidity and mortality. The guideline aims to identify which fall risk tools accurately identify patients at risk for falling in the ED. The evidence regarding fall risk assessment in the ED is limited, and most widely adopted tools have not been studied in this unique environment.<br /><br />The guideline discusses several fall risk assessment tools and their level of recommendation. For adult patients, the Morse Fall Scale, Johns Hopkins Fall Risk Assessment Tool, and Hendrich II Fall Risk Model can predict fall risk in inpatients. However, there is insufficient evidence to determine the translation of the Morse Fall Scale and the Johns Hopkins tool into Korean and Chinese, respectively. Additionally, the Hendrich II Fall Risk Model may not perform consistently across patient groups, skill levels, or units.<br /><br />Other tools such as the STRATIFY tool and the Humpty Dumpty Fall Scale have varying levels of accuracy in predicting fall risk. For pediatric patients, the CHAMPS tool and the Little Schmidy Fall Risk tool accurately predict fall risk, while adult fall risk tools like the Hendrich II and Morse Fall Scale are not specific or sensitive enough.<br /><br />In the ED specifically, the MEDFRAT tool can accurately predict fall risk in patients. However, there is insufficient evidence to determine the impact of individual characteristics, chronic illnesses, reason for ED presentation, and overall state of the ED on fall risk.<br /><br />The guideline provides a full description of each tool and its level of recommendation. It is important for clinicians to further validate the fall risk assessment tool within their own specialty unit or population. The guideline concludes that more research is needed to determine the most appropriate tool for accurately predicting falls in the ED.
